Affording the Dental Care Your Child Deserves
We strongly believe that all children should have access to dental care in an appropriate setting. Your child’s dental care will not be compromised for financial reasons. We offer a variety of payment options to accommodate any budget.
We accept most dental insurance plans and will work with you and your provider to maximize your benefits with our office. However, please remember it is your responsibility to be familiar with your dental insurance plan. For your convenience, we accept cash, check, Visa, MasterCard, and Discover. In addition, we offer financing with affordable monthly payments through Care Credit.
Payment is due on the date of service. If you have insurance, you will be expected to make an estimated payment for that portion not covered by your insurance plan.
Forms
New patients, please complete and submit the New Patient Registration electronically by clicking on the location where you are scheduled.
If you are unable to accompany your child to their appointment, you will need to complete The Consent for Treatment form. If you are leaving our practice and would like to have your child’s records transferred elsewhere, you will need to complete the Release of Records form. Neither of these can be submitted electronically and instead need to be printed and faxed, emailed, or mailed to the location you go to.

Tardy Policy
If you are going to be more than 10 minutes late for your appointment, we request that you call our office. If the schedule allows, the appointment time will simply be shifted to accommodate the delay. However, if the tardiness can’t be handled, we may request you reschedule your appointment. We work diligently to stay on schedule and suggest that you arrive 5 to 10 minutes prior to your appointment time to allow time for any necessary paperwork. Patients who are consistently tardy will be asked to seek dental care elsewhere.
Appointment Cancellation Policy
Your appointment time is reserved exclusively for you. We reserve the right to charge patients who do not reschedule with adequate notice, or who fail to show up for their scheduled appointments. Patients who have missed multiple appointments will be asked to seek dental care elsewhere.
To respect the needs of other patients, we require that you contact our office to reschedule at least 24 hours prior to your scheduled appointment time. Appointments are in high demand and your early cancellation will give another patient the opportunity to be treated. To cancel an appointment, please call our office. If you do not reach an office representative, you may leave a voicemail. You may not cancel an appointment via email.
